FILONEX-Study published: First Randomized Study of HDF during EVLP

Milestone in lung transplantation medicine: The FILONEX Trial is the first randomized controlled trial evaluating the additional use of hemodiafiltration (HDF) during ex vivo lung perfusion (EVLP) in marginal donor lungs.

This prospective randomized trial demonstrates that integrating HDF into the EVLP circuit is feasible and safe during six hours of perfusion, without adverse effects on EVLP performance or short-term outcomes after lung transplantation. In the treatment group, electrolytes, lactate, and pH remained within physiological ranges, while transplant acceptance rates were higher, with comparable short-term outcomes after lung transplantation.

These findings suggest that restoring physiologic clearance mechanisms during EVLP may improve organ quality and help enable longer perfusion times.
